About Harthill PCN
Harthill Primary Care Network (PCN) is a collaboration of six GP practices, led by a Clinical Director, working together to support a common goal : to improve the health outcomes for our patients
We know that our strength lies not only in the words we stand by but in the actions of our care teams in understanding our patient needs and working with them to deliver best possible care partnerships
Our GP practices agree that, by working across geographical boundaries, we can help our teams and our patients overcome challenges, improve lives and support more people, more efficiently and more effectively, with a shared passion and energy, to deliver the best possible health outcomes.
Who is Harthill PCN
The six GP practices who form Harthill Primary Care Network (PCN) all operate as individual, independent practices. The network is our opportunity to join forces in sharing best practice and innovation, combining experience and skills to the benefit of all our patients
Our Clinical Director is a practising doctor whose role is to guide and lead the practices in reviewing current provision as well as driving service redesign and delivery. The work of the Clinical Director includes supporting our practices in developing community focused services and forging closest possible links with hospital, social and community care providers to improve patient pathways on their health journey.
The role does not mean the Clinical Director is responsible for how network practices are run or how they provide services to their individual patients : that remains for the individual practice.

What Harthill PCN is doing
Much of what our network does is build and maintain links with our members practices to ensure we can support each other and develop services that work across the area we cover. Three examples which are of particular focus to us during 2021 are shown below. We have also made available a range of self-help / self-care booklets and links to support patients with their own healthcare. You can find out more about these in the Help Yourself to Network Services section of our website.
Enhanced health in Care Homes
Of particular focus is our work establishing enhanced care home support that provides improved access for residents to a healthcare visiting service.
Led by a paramedic and backed up by a pharmacist team we aim to reduce the time taken by individual care homes to get in touch with a healthcare team
Supporting this service we are focusing on developing a medication optimisation service
This is a major development that will ensure our care homes are able to access the best support for their residents
Bereavement advice and support
Working with colleagues at Humber NHS Trust and using the expertise across our network, including input from Social Prescribers, we have developed a Bereavement Protocol that has been adopted across our network surgeries.
This aims to ensure bereaved families, friends and carers have access to timely information and advice when a death occurs - and in the weeks and months afterwards, when the realisation of losing a loved one can sometimes become unbearable, we have supportive help available to help with loneliness, financial difficulties or any other issues that can arise as a result of losing someone close.

Carer advice and support
Alongside our work developing the bereavement protocol we more fully understood that many of our patients are carers - not necessarily by occupation but carers for family or friends - and that many of these carers were either not recognising themselves as carers and/or not all of them were accessing any support for themselves in that role.
Our Carer Support Protocol has been developed with a lot of input from East Riding Carer Support Service and has resulted in each of our surgeries appointing and training a Carer Champion(s) whose role is to be available to any patient who is or becomes a carer.
There is a range of support that can be accessed - be it as simple as some-one to talk to right through to arranging respite care or help with medication or even understanding what financial support is available,
Each carer and each circumstance is unique and individual. If you care fro somebody you can contact any of our practices and ask to speak to the Carer Champion - they know what and where to guide you for help and support

Harthill PCN does not have a physical location so we will use this website to inform patients of the work we are doing and create links to network services our practices offer as these develop
Feedback
We are developing a feedback form (see example) so that if you have a message or a question for the network we want to be able to hear from you.
This form is not yet set up and operational - we will update this area once the contact service is live : expected by end 2020
Please note: this form must not at any time be used for any personal or medical queries - nor as a way to communicate with or about your GP practice
Such queries must always be directed to your own surgery
Harthill PCN, as a network, has no responsibility for the services or the care given by individual surgeries and has no right of influence in the individual patients services delivered by them.
Your own practice team will be the ones able to help you with such queries and they are always ready to help and support patients.

Help yourself : a range of services you can access to support your healthcare
There are many ways people can take control of their healthcare - and many of which do not always need you to visit a nurse of doctor,
Looking after your mental health is just one of those areas where there are many resources you can access - either online or by telephoning a specialist service.
Carer Support Services, Exercise and Fitness, Weight Management : these are just some of the areas where patients can find information and support at their own pace and Harthill PCN wants to make as many of these as possible available to our patients through this website.
